onehome set out to solve a breakdown in a healthcare system that prevents patients from receiving fully coordinated care at home. More than ever, patients want to heal at home, and with 10,000 baby boomers turning 65 every day, demand is only growing. At the time of hospital discharge, too many people are sent to skilled nursing facilities for
non-medical reasons, or sent home with inadequate care, only to quickly return to the ER or hospital. This failure stems from misaligned payment models, poorly coordinated home-based services, and a lack of focus on quality in short, from a system that doesn’t work for today’s and tomorrow’s – patients. It affects parents, grandparents, friends, and, one day, us. It’s what has led onehome to change the face of post-acute and home-based care.
