discharges to reduce readmissions, and avoids network leakage caused by out-of-network referrals; ICW Care Management, a Web based solution that allows processes to be modeled on the basis of guidelines, intervention catalogs, and organization-specific care pathways; and ICW Patient Engagement that gives access to the patient’s longitudinal record by enabling data-driven electronic conversations and allowing bilateral assignment of health care activities.
InterComponentWare’s solutions also comprise ICW Health Information Exchange for risk-based contracting and cooperative care delivery scenarios; ICW Master Patient Index that compares patient records from various information sources and determines if they are referencing the same patient or not; and ICW eHealth Technology that enables interface and adapter implementation, and ensures that products are integrated into the customer’s environment. It serves health information exchanges, care delivery organizations, payers and health plans, and life science customers in the United States, Germany, and Switzerland.
InterComponentWare began operation in 1998. It has its headquarters in Walldorf in Germany with a subsidiary in the United States.
