What we do: For payers and providers struggling to optimize margins and achieve the Triple Aim, Care at Hand provides a smart survey platform that identifies early medical and psychosocial risk
factors for hospitalization.
What makes us unique: Unlike episodic predictive models limited to claims and EMR data, Care at Hand predicts hospitalization risk in the blind spot between doctor visits using the observations of existing frontline staff.
Evidence: $9,056 Medicare A savings per beneficiary per year (Avalere 2015). 39.6% reduction readmissions & 257% ROI (AHRQ 2014). Predict admissions up to 120 days (PHIM 2016).
Business Model: We’re a SaaS business. We license our software to payers and providers that 1) seek to increase revenue from outcomes-based reimbursement and 2) leverage existing low-cost, high touch-frontline staff. We enjoy taking financial risk for our outcomes.
